1. Complete the insurance claim form if possible within 24 hours, and latest within 7 working days and hand it back to the office for processing.
2. Attach a photocopy of your Insurance Card, Identity Card and Membership Card.
3. Submit repair bill
4. Take a photo of the club before and after repair.
5. For the case of a stolen club or accident, attach a police report to support your insurance claim.
6. For Hole-In-One claim, submit a Certified True Copy of your score card, stamped and sign by the respective Golf Club where your Hole-In-One is achieved.
